College of Surgeons guidelines for Surgical Assistant need

base their denials on this document and Medicare's policy

Surgical
Assistant services patient bill
|

form to send a bill to the patient if the insurance company
has denied payment because of a non-covered service or if
it applied to deductible, co-payment, etc. Do not send this
letter if the insurance company has denied payment because
it considers the service as a bundled service, this issue
should be handled with the insurance company directly or the
